Pure Healthcare is looking for a committed professional to join our team as a Community Health Worker and Outreach Coordinator. What You Should Know About the Community Health Worker and Outreach Coordinator Role: This is a Full-Time position, Monday-Friday, 8am-5pm The Essential Duties Are: You will work alongside other members of our clinical team, providing a multi-disciplinary approach to care for our members with serious illness and ensuring the delivery of high-quality supportive care services to our patients, their families and community providers. Qualifications: Minimum 1-year health care experience and understanding of the person-centered care philosophy, Care Coordination and/or Managed Care setting strongly encourage.
