•  Provide patient care per care plans and assist with daily activities.

•  Report patient status changes and document care accurately.

•  Qualifications include prior Home Health Aide experience and Indiana CNA certification preferred.

•  Must have valid driver s license, auto insurance, pass background and drug tests, and TB testing.
