Position Summary
The Clinical Transition Nurse (CTN) plays a key role in the transition and admission of patients into hospice care. This position involves planning, assessing, and coordinating patient care in the home or residential setting. As a CTN, you will serve as both a clinical resource and a compassionate guide for patients and families navigating end-of-life care.
Key Responsibilities
Conduct comprehensive assessments of patients and families during transitions into hospice services
Develop individualized care plans in coordination with the interdisciplinary team
Administer medications and perform prescribed nursing procedures
Provide patient and family education related to the hospice care journey
Supervise hospice aides and ensure quality and safety in delegated care
Serve as the clinical point of contact for families, referral sources, and internal teams
Maintain timely and accurate documentation that complies with regulatory and organizational standards
Required Qualifications
Graduate of an accredited nursing program
Active RN license (California)
Current CPR certification
1-2 years of recent acute care experience
1-2 years of recent home care experience
Valid driver's license and insured, operational vehicle
Physical ability to lift, position, and transfer patients and to perform prolonged walking/standing
Strong clinical assessment, planning, and documentation skills
Ability to work independently and collaboratively in the field
Preferred Qualifications
Bachelor's degree in Nursing (BSN)
At least 1 year of experience in hospice care
Exposure to holistic, complementary, or alternative care modalities
