Job Title: Clinical Supervisor - Registered Nurse (RN)


Position Overview:


We are seeking a Clinical Supervisor to manage the clinical operations of our home health services. This position requires an experienced Registered Nurse (RN) who will work under the supervision of the Director of Clinical Services or Executive Director. The Clinical Supervisor will oversee the daily operations of the branch office, including managing staff, ensuring high-quality care, and maintaining efficient delivery of home health services in compliance with company policies, procedures, and time management standards.


The Clinical Supervisor will also develop, implement, and evaluate goals and objectives to meet the highest quality standards and contribute to the organization's overall mission. This role involves active patient, caregiver, and family counseling and education, as well as communication with physicians and other healthcare professionals regarding care plans. The RN will assist with clinical care, revise care plans in collaboration with patients and caregivers, and perform field visits as necessary.


Key Responsibilities

  • Directly oversee the daily clinical operations of home health services and manage assigned staff to ensure efficient and effective patient care delivery.
  • Develop, implement, and evaluate clinical goals, ensuring they align with agency standards and regulatory requirements.
  • Act as a liaison between patients, caregivers, physicians, and other healthcare providers to coordinate comprehensive care.
  • Participate in patient, caregiver, and family education and counseling sessions to support treatment goals.
  • Conduct ongoing interdisciplinary assessments of patients and contribute to the QAPI (Quality Assurance and Performance Improvement) program.
  • Supervise, mentor, and direct nursing personnel, providing guidance and support.
  • Periodically perform field visits and administer skilled nursing care to patients as per the care plan.
  • Support agency leadership by potentially serving in alternate administrative roles (Administrator, Alternate Administrator, Director of Nursing, or Alternate Director of Nursing).

Qualifications & Requirements

  • Licensure: Currently licensed as a Registered Nurse (RN) in the state of practice.
  • Experience: Minimum of three (3) years of nursing experience, with a preference for home health experience.
  • Certifications: CPR certification from the American Heart Association or American Red Cross.
  • Skills & Abilities:
  • Strong clinical management and leadership skills.
  • Ability to supervise, mentor, and guide nursing personnel.
  • In-depth knowledge of Medicare regulations and accreditation criteria.
  • Excellent communication, critical thinking, and problem-solving skills.
  • Ability to collaborate effectively with interdisciplinary teams and work independently.
  • Strong customer service orientation and interpersonal skills.
  • Additional Requirements:
  • Must have a current driver's license, auto liability insurance, and reliable transportation.
  • Availability for on-call duties at least once per month, or as needed.
  • Travel within the service area may be required.

Education Requirements

  • Education: High School Diploma or GED required. Graduate of an approved school of nursing.

Working Conditions & Physical Requirements

  • Work Environment: Primarily office-based with travel to patient homes as necessary.
  • Physical Effort: Requires moderate physical effort including kneeling, squatting, reaching, walking, and occasional lifting (up to 100 lbs with assistance).
  • Ability to adapt to varying environmental conditions, such as temperature and humidity changes.
  • Exposure to various conditions, including bodily fluids, hazardous materials, and potentially unpleasant odors.

  • Monday - Friday, 08:30 AM - 05:00 PM
