Clinical Supervisor

The RN Case Manager provides licensed nursing care, developing and evaluating patient-centered care plans under physician supervision.
•  Manages assigned patients, conducts evaluations, and coordinates care with families and healthcare teams.
•  Documents accurately per regulatory standards (OASIS, PAR) and promptly reports changes.
•  Acts as patient advocate, educates families, and administers medications following policy.
•  Participates in quality assurance, reports incidents, and maintains confidentiality.
•  Provides on-call support, supervises staff, and ensures documentation within required timeframes.
•  Requires a nursing license, relevant experience, computer skills, and ability to travel in Evansville, IN.
•  Full-time position offering benefits, with a schedule including on-call shifts.
