We are seeking experienced Inpatient Pharmacists to join our healthcare team in a hospital setting in New Jersey. The ideal candidates will possess strong clinical knowledge, a deep commitment to patient care, and experience working in an inpatient pharmacy environment.
Key Responsibilities:
Provide pharmaceutical care and services to inpatient patients, including medication therapy management and drug information support.
Conduct medication reconciliations for patients, ensuring accuracy and safety in the medication process.
Prepare and dispense medications according to physician orders in compliance with state and federal laws.
Monitor and review drug therapy for efficacy, safety, and potential interactions, making recommendations to healthcare teams as necessary.
Ensure compliance with USP 797 and USP 800 guidelines in the IV room, including preparation of sterile compounded medications.
Collaborate with healthcare providers to optimize patient care, including medication management and therapy adjustments.
Educate patients and healthcare team members on the safe and effective use of medications.
Provide support in the management of chemotherapy medications, including preparation, dispensation, and patient counseling.
Maintain accurate and up-to-date records of medications and treatment plans.
Qualifications:
New Jersey State Pharmacist License required and in good standing.
BLS (Basic Life Support) certification required.
Chemo and IV Certification required.
Must have experience in USP 797/800 IV room practices for sterile compounding and safety.
Proven experience in medication reconciliation and inpatient pharmacy settings.
Must be a Registered Pharmacist (RPh).
