Registered Nurse to implement and oversee percutaneous valve program in its entirety by working closely with cardiac surgeons, interventional cardiologists and non-invasive cardiologists as well as other members of the health care team. Responsible for medical record abstraction, data entry, validation, and report generation for the STS clinical data bases related to both cardiac surgery and thoracic surgery. Presents findings and generates reports as requested. Serves as a resource for the cardiothoracic PI team and cardiovascular PI program. Helps facilitate the care and education of cardiac surgery patients through the continuum of care.

ESSENTIAL FUNCTIONS:

  • Implements percutaneous valve program in its entirety by working closely with cardiac surgeons, interventional cardiologists and non-invasive cardiologists as well as other members of the health care team.
  • Networks with other health systems that were part of the "Partners" trial to identify processes related to patient identification and care delivery.
  • Establishes processes with cardiology practices that leads to the identification of potential patients.
  • Educates staff (practice staff, inpatient team, technical staff) concerning the overall program and specific patient and family care needs.
  • Coordinates physician educational sessions for both physicians (referral base) and the public.
  • Schedules patients for surgery.
  • Confirms surgery schedule with Edwards and the operating room team and coordinates proctor and Edwards's staff.
  • Works with OR team to ensure needed equipment is available.
  • Works with physician liaisons to develop and implement educational program for referring physic
  • Works with St. Luke's Information Technology team to establish the Intelemage system from Edwards that is used to evaluate and track patients' pre-post-surgery.
  • Enters and maintains patient information into Intelemage system.
  • Works closely with the Edwards team to process potential candidates and schedule procedures in the future.
  • Serves as the point of contact for the overall valve program.
  • Coordinates team (cardiac surgery, interventionalists, Interventional Radiologists, Non-invasive cardiologists) meeting to evaluate patients.
  • Maintains TAVR registry (enters initial data and maintains patient information and outcomes).
  • Coordinates post hospitalization visits, follows up with patients post hospitalization, and maintains contact with patients and family members.
  • Interviews potential candidates and coordinates all patient pre-screening activities (TEE, Cath, CTA, PFT etc ).
  • Maintains a current knowledge of the abstracting guidelines, guidelines for data entry and specification manuals updates and revisions for the STS and Thoracic Aortic data base.
  • Harvests data and submits to external clinical registries.
  • Completes chart abstraction and date entry as well as report generation
  • Maintains a current knowledge of computer software upgrades and applications.
  • Acts as technical support liaison between Cardiopulse and St. Luke's Hospital.
  • Identifies and reports problematic areas related to medical record documentation.
  • Serves as the primary contact to process internal and external data requests related to Thoracic Aortic data.
  • Actively participates in the Performance Improvement functions.
  • Analyzes information and updates various graphs, charts and tables using appropriate data presentation and analysis.
  • Performs monthly audits, reports, and queries for each data base.
  • Helps coordinate the implementation of new clinical information software, including installation and structuring of clinical databases, establishing data entry, auditing and reporting procedures for these databases.
  • Assist in outpatient clinics, with performance of vital signs, weights, EKG, supervised evaluation of patients, patient education where appropriate, and completion of insurance and disability forms.
  • Serves as telephone contact for patients/families and VNA for the Cardiothoracic Surgery program.
  • Works with physician, physician assistant, and/or nurse practitioner as directed.
