Position Overview

The Clinical Education Manager (LVN/LPN) is responsible for providing ongoing education, training, and clinical compliance coaching at Skilled Nursing Facilities (SNFs) and home health agencies. This role emphasizes delivering in-services, continuous staff development, and regulatory compliance training focused on CMS Quality of Care standards (F684) and Pressure Ulcers/Injuries management (F686).

You'll collaborate closely with clinical staff, MDS Coordinators, and medical providers to ensure accurate documentation, consistent training outcomes, and compliance with regulatory requirements-directly impacting clinical excellence and reimbursement accuracy.

CMS Regulatory Focus (F-Tags):
F684 (Quality of Care): Ensuring each resident receives care that promotes achieving or maintaining their highest practicable well-being, including comprehensive wound prevention and care.
F686 (Pressure Ulcers/Injuries): Educating facilities on preventing avoidable pressure injuries, providing accurate assessment, proper treatment, documentation, and monitoring of existing wounds.

Key Responsibilities

Clinical Training, In-Services, & Continuous Development
Deliver structured clinical education sessions and in-services focused on wound staging (Stages 1-4, DTI, unstageable wounds), chronic wound management (pressure ulcers, diabetic wounds, venous wounds), and compliant documentation practices.
Provide continuous education and professional development support, identifying ongoing training needs through audits, surveys, and facility feedback.
Educate facility nursing staff on documenting advanced wound treatments (e.g., debridements, skin grafts) within LVN/LPN educational scope.
Develop, customize, and deliver comprehensive training materials, virtual education sessions, skills labs, and mock surveys.

Documentation Audits & Regulatory Compliance
Conduct regular clinical documentation audits (Preliminary Wound Reports, Skin Sweeps, GT Sweeps), ensuring adherence to CMS guidelines (F684, F686).
Provide targeted corrective education based on audit findings, reinforcing accurate documentation essential for MDS accuracy and PDPM reimbursement.

Facility & Provider Collaboration
Serve as the primary documentation liaison between facility nursing teams, MDS Coordinators, and medical providers (MDs, NPs, PAs).
Professionally address and resolve documentation discrepancies, clearly reinforcing CMS regulatory expectations.

Compliance & Professionalism
Maintain consistent compliance with HIPAA, CMS regulations (F684, F686), state-specific guidelines, and patient confidentiality standards.
Demonstrate professionalism, integrity, and composure during regulatory audits, state surveys, and clinical interactions.

Qualifications

Required
Active, unrestricted LVN/LPN license in state of employment (WA, TX licenses strongly preferred)
Diploma or Associate Degree from accredited Practical/Vocational Nursing program
3+ years direct wound care experience (SNF, LTC, home health)
1+ year clinical education, training, or compliance auditing experience
Advanced knowledge of wound staging, chronic wound management, and CMS guidelines (F684 & F686)
Valid driver's license, reliable transportation, and willingness for frequent regional travel
Proficiency with Electronic Medical Records (EMRs), mobile documentation platforms, Google Workspace, and audit software
Exceptional communication, public speaking, and interpersonal skills

Highly Desirable
Direct experience collaborating with SNFs and MDS Coordinators on MDS accuracy, PDPM reimbursement, and regulatory survey preparation

Preferred
Wound Care Certification (WCC, WOCN, CWS)
Experience with CMS/state regulatory audits, QAPI processes, and multi-site clinical training

Technical & Physical Requirements
Skilled with mobile technology, EMRs, virtual conferencing platforms, and clinical audit software
Physically capable of frequent standing, walking, stair climbing, and safely transporting training materials (up to 25 lbs)
Comfortable independently navigating diverse clinical environments, including facilities with limited ADA accessibility
