Clinical Coordinator

Tallavera is looking for a Registered Nurse (RN) or a Licensed Practical Nurse (LPN) for a Clinical Coordinator position in a well known Long Term Care facility in Rochester, NY. The Clinical Coordinator plays a key leadership role within the rehabilitation team, supporting nursing staff and ensuring high-quality, resident-centered care. This position focuses on improving clinical outcomes by coordinating care plans, collaborating with providers, and guiding daily clinical operations.

Responsibilities of the Clinical Coordinator (RN or LPN):
Coordinate clinical services for all residents in the rehab unit
Oversee individualized care plans and ensure follow-through by nursing staff
Assign staff based on care needs and ensure effective communication
Provide emotional and social support to residents and families
Assist nurses with bedside care and participate in audits and floor rounds
Foster a culture of compassion, responsiveness, and teamwork

Qualifications of the Clinical Coordinator (RN or LPN):
Active NYS RN or LPN license required
Minimum 1 year of experience in long-term or acute care
At least 1 year of formal healthcare leadership experience
Rehabilitation experience strongly preferred
Current CPR certification required
Strong communication and leadership skills
Must meet NYS Department of Health immunization requirements

Job Type: Full-time

Pay: Up to $100,000.00 per year

Schedule:
8 hour shift
Day shift
Rotating weekends
