SUMMARY: The Clinical Case Manager provides case management and community psychiatric support treatment within a designated cottage, collaborating with clinical and operational teams to implement therapeutic recommendations. ESSENTIAL DUTIES:
Maintain a team-first spirit and effective communication. Complete logs, reports, and treatment plans on time. Facilitate group sessions and individual meetings with residents. Attend relevant meetings and appointments for client support. Manage cottage calendar and coordinate client communications. QUALIFICATIONS: Requires a Bachelor's degree in social work or related field, excellent communication skills, and a valid Ohio Driver's license. Candidates must be 21 or older and pass background checks.
