Clinical Care Manager

Location: Worcester, MA - Behavioral Health Partners MetroWest

Pay rate: $70,000-$75,000/annually

Schedule: Monday - Friday 8:30am - 5:00pm (Must be able to work some early mornings)

The Clinical Care Manager is responsible for:

Collaborating with team members to improve care quality and reduce episodic care.

Supervising care coordinators to assist MassHealth members.

Working with a multidisciplinary team to coordinate care for clients with complex health issues.

Participating in community partner meetings.

Supporting care coordinators in client advocacy.

Developing and updating person-centered treatment plans.

Conducting weekly supervision with care coordinators.

Ensuring compliance with BHCP policies and training.

Qualifications:

Masters degree with first level license or relevant experience.

Strong communication, collaboration, and customer service skills.

Excellent interpersonal and organizational skills.

Proficient in Windows operating systems and Office Suite.

Valid driver's license and access to an insured vehicle.
