Employment Type: Full time Shift: Description: Provides diabetes education and training for clients of MercyOne Northeast Iowa. Assists with the development of diabetes services. Adheres to MercyOne Northeast Iowa performance expectations and performs all defined services and other related duties in accordance with the mission, vision and values of MercyOne Northeast Iowa. What you will Do: Provides assessment, teaching and follow-up instruction to program participants in individual and group settings. Receives referrals and order from physician Conducts initial needs assessment Provides education appropriate to individual (i.e., age, reading level, language, disability) Coordinates/provides follow-up services in person or via phone. Receives and returns calls from patients in a timely manner. Hours/Schedule: 64 hours biweekly (.8 FTE) Minimum Qualifications: Minimum of 2 years medical/surgical nursing experience; patient education and diabetes experience preferred.
