The state supported living centers serve people with intellectual and developmental disabilities who are medically fragile or who have behavioral problems. Clerk I performs routine medical records clerical work in order to support the Individual Records functions. Work involves securing legal documents and Medical Records; handling and transporting medical records; filing, sorting and retrieving legal reports; and protecting the confidentiality of Individual's records. Clerk I works under moderate supervision of the Individual Records Manager with limited latitude for the use of initiative and independent judgement.

Essential Job Functions (EJFs):

Attends work on a regular basis and may be required to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned.

Sorts, alphabetizes and files legal documents into the centralized Active Medical Record to provide all available information for review within two (2) working days.

Purges documents from active medical records according to the department Active Record Order Guidelines (AROG) and retention schedule.

Performs qualitative and quantitative record audits to identify timeliness and quality of documentation using both the analysis tool provided by Unified Records Department, the facility Electronic Health Record (EHR), and IRIS (Integrated Resident Information System).

Prepares inactive documents and inactive medical records for off-facility storage by following the HHSC State Supported Living Center storage guidelines and procedures.

Initiates and assembles medical records for new admissions, return from community placement and transfers with reassignments to provide a complete history of services.

Reviews medical record content quarterly to identify any misfiles in Active Record and that the appropriate ISP (Individual Support Plan) to ISP (Individual Support Plan) purging has been performed according to AROG.

Mends and repairs pages in the medical record to protect the preservation of legal documents.

Prepares, assembles and prints from facility EHR, Death Record Review Files in chronological order within fifteen (15) working days of expiration for committee review, using the Death Record Review Files templates.

Provides clerical support to main office in absence of the Director to ensure daily operations of the Medical Records Department.

Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency's obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Such participation may require an alternate shift pattern assignment and/or location.

Knowledge, Skills and Abilities (KSAs):

Knowledge of general records management or medical records office procedures.

Ability to transport medical records.

Ability to maintain files.

Ability to use office machines.

Skills in computer applications.

Ability to communicate with internal and external customers.

Registrations, Licensure Requirements or Certifications:

None

Initial Screening Criteria:

High school diploma or GED required.

One year of general records management or medical records experience required. Two or more years of general records management or medical records experience preferred.

Experience with use of medical terminology.

Additional Information:

Applicants must pass a fingerprint criminal background check, pre-employment drug screen, and registry checks including the Client Abuse/Neglect Reporting System (CANRS), Nurse Aide, Medication Aide and Employee Misconduct and HHS List of Excluded Individuals/Entities (LEIE).

All State Supported Living Center Employees are subject to Random Drug Testing.

Flexibility in work hours may be required for this position. The position may be required to work overtime and/or extended hours. Must be willing to work weekends and holidays.
