JOB TITLE: CHR Transporter and Assistant

(Rossville, KS Part-time)

ORGANIZATIONAL UNIT: Health Services

Department: Senior Support Network

SUPERVISOR: SSN/CHR Coordinator

FLSA: Non-exempt

SALARY: BOE $9.00-16.00

JOB SUMMARY: Transporting Tribal Elders to medical appointments or other destinations as needed by the Senior Support Network. Offering assistance to the Elders as needed to get to their appointments. Follow the CPN CHR Transport policy. Assist the CHR nurse as needed.

CPN Health Services personnel may be assigned to provide services to patients of all ages including but not limited to Indian Children and will require a criminal history background screening.

ESSENTIAL FUNCTIONS:

• Provide Tribal Elders safe transportation to and from medical appointments or other destinations as required.

• Give assistance as needed to get the Elder to their appointments-this may include helping them with their assistive devices or with any paperwork they are given at their appointment.

• Document all mileage, addresses, and client signatures as required on Transport Logs.

• Promptly report any problems encountered with the SSN Supervisor.

• Provide friendly and courteous customer service.

Performs assigned supportive procedures for the well-being of tribal elders.
  • Provide patient advocacy and case and resource management.
  • Assist with personal care/Activities of Daily Living for patients: nutrition, shopping, running errands, and minor house cleaning,
  • Provide support to CHR nurse as needed.

ADDITIONAL DUTIES:
  • Perform other duties and projects as required by management and provides support as needed and defined by the CHR nurse.
  • Make home visits ordered by physician or SSN Project Director.
  • Participates in the community health promotion and disease prevention events.

KNOWLEDGE, SKILLS, AND ABILITIES:
  • Ability to communicate effectively and accurately with patients and staff.
  • Ability to maintain strict confidentiality regarding all patients encounters.
  • Computer skills
  • Specialized knowledge, skills, and abilities to successfully perform duties of the position and to recognize important status changes with the elder patient and report accordingly.

MINIMUM QUALIFICATIONS FOR CONSIDERATION:
  • Certified or willing to be certified as CHR upon request within one year of employment.
  • Certified in Basic Life Support (CPR) or becomes certified within 3 months of employment.
  • Must have a valid driver's license and driving record that allows access to CPN vehicles for daily work function.
  • Computer skills required for documentation in the patient record.
  • Must have a High School diploma or GED equivalent
  • Must have a good driving record and personal automobile insurance
  • Must have very good communication and written skills
  • Ability to work a varying schedule as needed.

PHYSICAL REQUIREMENTS:
  • Sitting or standing or walking for long periods of time; occasional bending, squatting, kneeling, stooping; talking, hearing, and visual acuity
  • Frequent lifting (up to 50 lbs) and/or occasional lifting (up to 75 lbs)
  • Good driving skills

ENVIRONMENTAL CONDITIONS:
  • General clinical and home environment with frequent exposure to chemicals, blood borne pathogens and patient body fluids.
  • Varying environments such as medical offices, hospitals, homes, and other environments; differing vehicles depending on transport needs.
