Charge Nurse

•  The Registered Nurse Case Manager provides hospice care, coordinating and directing services for patients and families at home. Responsibilities include comprehensive patient assessments, developing and revising care plans, administering medications, and providing health education. The RN coordinates with physicians and the care team, supervises hospice aides, maintains accurate records, and participates in on-call rotation.
