Scope Of Services: - Provide CNAs to support students (ages 3-22) with special needs in daily living skills such as toileting, personal care, and feeding. - CNAs will be needed during the school year (181 days) and for extended school year services (20 additional days in summer, if necessary). - CNAs must be licensed and certified in Connecticut. - CNAs must provide their own transportation to the assigned school; parking expenses may be reimbursed by the proposer. - CNAs must undergo and pass background checks, drug tests, and DCF registry checks. - CNAs must maintain service notes in accordance with Connecticut nursing practice standards (both handwritten and electronic formats). - CNAs must comply with all relevant federal, state, local, and district laws, policies, and ethical standards. - The proposer must assign at least one designated contact person to handle staffing, issue resolution, and communication with the City. - The proposer must monitor service quality, ensure accurate time billing, and provide service notes upon request for IEP-related matters. - The proposer must submit invoices no later than 30 days after services are rendered, with all necessary supporting documentation. - The proposer must provide in-service professional development and relevant nursing materials to Waterbury staff, parents, and public health program clients if requested. - If a CNA is unable to complete their shift, the proposer is responsible for finding a substitute.
