WHAT YOU WILL DO: This position, under the supervision of the registered nurse, will serve clients who have been discharged from local area hospitals including the VA Medical Center or referred through their managed care plan to achieve a more adaptive level of physical, psychological and social functioning while in the program. Working within a comprehensive team of skilled professionals the CNA engages with the recuperative care team to get the patient back to self-sufficiency.
- Monitor clients to ensure their health, safety and welfare through conducting daily rounds or by appointment as needed
- Perform basic medical procedures, including vital signs, breathalyzer and collection of specimens for urine drug screens
- Assist clients with activities of daily ADLs and instrumental activities of daily living (IADLs)
- Ensure facilities are maintained in clean and orderly fashion, including bedrooms, bathrooms, hallways and common areas preserving a safe environment for all clients
- Orient new clients to their room, facility, laundry, dining area, common areas etc.
- Assist clients with scheduling medical or mental health appointments and any transportation necessary
- Document all client interactions and participate in weekly case file review
- Ensure the confidentiality of patient information and health records
- Inform the Registered Nurse and Licensed Vocational Nurse of any changes in client condition
- Participate in all staff meetings and trainings
- Perform additional duties as assigned by supervisor
EDUCATION / EXPERIENCE- High School Diploma or GED equivalent required
- Must be currently certified by the State of California as a Certified Nursing Assistant
- 1 year experience preferred
- Knowledge of substance use disorders preferred
- Experience working with severely mentally ill, medically fragile, or homeless populations preferred
Schedule: Tuesday-Saturday 8 am - 4:30 pm
