As a Certified Home Health Aide (CHHA/CNA) for the Hospice team, you'll support patients in their homes with critical end-of-life care. You will be an essential member of a multidisciplinary care team, helping patients maintain dignity and comfort while receiving compassionate, respectful care.
What You'll Do
Assist patients with personal care needs, including:
Bathing, back rubs, oral hygiene, shampooing, and dressing
Skin care to prevent breakdown
Toileting activities and incontinence care
Help patients with the self-administration of medications as instructed
Change bed linens and maintain a clean, orderly living environment
Observe, document, and report changes in the patient's condition
Collaborate with clinical staff and contribute to the patient's overall care plan
Follow all agency documentation standards and maintain professional conduct
Meet daily productivity expectations as determined by business needs
What You'll Need
Valid California certifications as both:
Current Basic Life Support (BLS) and CPR certification
Valid driver's license with reliable, insured transportation
Strong personal hygiene, professionalism, and empathy for patients in hospice care
Self-motivated, compassionate, and capable of working with minimal supervision
