Position Summary

The Home Health Aide (CHHA) - CNA is responsible for providing personal care and support services that promote comfort, safety, and independence for patients in their homes. This hands-on role plays a vital part in ensuring continuity of care, especially for vulnerable or recovering patients. You'll serve patients across a rotating territory and provide compassionate, direct assistance under the supervision of licensed clinicians.

Key Responsibilities

  • Assist patients with personal hygiene: bathing, oral care, shampooing, and skin maintenance

  • Aid in dressing, toileting, and mobility-related needs

  • Help patients self-administer medication as instructed

  • Maintain clean and orderly living environments

  • Report changes in patient condition to the supervising nurse

  • Document care provided accurately and in compliance with agency standards

  • Follow established care plans and uphold safety protocols at all times

Required Qualifications

  • Active Certified Nursing Assistant (CNA) certification

  • Certified Home Health Aide (CHHA) credential (Oregon)

  • Current CPR certification

  • Must be at least 18 years old

  • Ability to read, follow written instructions, and document care accurately

  • Valid driver's license, insured and reliable vehicle in good working condition

  • Meets federal and state training requirements for home health aides

  • Able to work independently with minimal supervision

Territory & Schedule

  • Per diem role with flexible scheduling and growth opportunity to full-time

  • Daily travel expected between Salem, McMinnville, Lebanon, Albany, and Corvallis

  • Willingness to commute across a 60-mile service radius required
