Rural Health Group is seeking a full-time Registered Nurse to fill the role of Certified Diabetic Educator at one of our outpatient primary care clinics located on the "west side" of our service area. The clinics in this region include: Littleton, Norlina, Hollister, Henderson and Stovall. You will have the opportunity to indicate which location(s) you are interested in on the application.
The Certified Diabetic Educator (CDE) is a key member of the health care team and works closely with providers, clinic management, nursing staff and case management. The CDE possesses clinical expertise in care related to diabetes management and education and other chronic diseases.
Essential Duties/Responsibilities
• Provide diabetes and self-management guidance and education to pre-diabetics, type 1, type2, and gestational diabetic patients. Develop individualized diabetes education plans for patients in collaboration with the medical provider, and monitor adherence to plan. Individualized education plans shall take into account different cultural, physical, cognitive, and literacy levels of patients and families and meet the evolving needs of the patient.
• Plan, develop, and implement individual and group diabetes education strategies for patients through their life span and their families and caregivers.
• Provide nutritional counseling for weight control and diabetes management. Use comprehensive knowledge of nutrition to assist patients with meal planning. Teach patients to manage blood glucose with carbohydrate counting and to manage weight with calorie counting. Monitor patient diet diaries.
• Participates in Quality Improvement efforts by conducting chart audits and by monitoring various aspects of the program
• Assist medical provider in assessing the need for, and frequency, of self glucose monitoring, and teach patients self glucose monitoring. Monitor adherence to the plan. Assist medical provider in evaluating blood glucose logs in conjunction with diet diaries at provider visits.
• Instruct patients and family members on medication safety including preventing, recognizing, and treating hypoglycemia. Teach how to administer insulin. Assist medical providers in adjusting oral medication and insulin doses according to blood glucose measurements, dietary habits, and individual barriers to care.
• Work with patients and the care team to ensure patients have access to medications and ensure patients keep appointments with medical provider and CDE for diabetes monitoring.
• Identify individual social and emotional barriers (such as depression) to compliance with the treatment plan, and refer patients to the appropriate source for care.
• Assist provider in monitoring patient's diabetes care flow sheet, ensuring patients get timely recommended preventive and monitoring tests, treatments, and referrals per diabetes care guidelines.
• Facilitate coordination of care across specialty care, facility-based care, and community organizations.
• Assist in the review and/or the development of clinical practice guidelines. Serves as a content expert for diabetes education and management.
• Assist in the development of policies and procedures for diabetes and health education guidelines.
• Support the organization as needed.
• All other duties as assigned.
Qualifications
REQUIRED:
• Bachelor of Science in Nursing and/or Dietetics from an accredited facility is required.
• Licensed as a Registered Nurse or a Registered Dietitian is required.
• Certification by the National Certification Board for Diabetes Educators (CDE) is required.
• Must have experience with diabetic education and case management, low-income families, diverse communities, community/migrant health, patient education and computer skills.
Preferred:
• Three years of experience caring for individuals with diabetes
• Bilingual in Spanish is highly preferred.
Must obtain a CPR and First Aid Card within ninety (90) calendar days of employment and maintain valid cards during employment.
