Function

Reporting to the Director of Programs, the Case Specialist provides outreach and case management for program participants and is responsible for the recruitment, review of eligibility requirements, and career services of participants in various programs.

Duties & Responsibilities

  1. Conduct outreach to, orient, and enroll participants in various programs;
  2. Manage referrals, process documentation, and provide culturally proficient services/case management to program participants promptly (attend to diverse needs, including but not limited to language needs, etc.);
  3. Assist prospective participants with completing the required eligibility applications and other forms;
  4. Provide wraparound supportive services, tailored career development assistance, and proficiency exam preparation to participants.
  5. Maintain accurate and complete records of important documents and data in electronic databases and hard copies in filing cabinets, per program regulations and guidelines, as well as HIPAA;
  6. Ensure accurate and proper maintenance of case files by maintaining attention to details, frequent review, and updating of files and documents provided by participants;
  7. Ensure up-to-date contact with program participants;
  8. Identify resources available to participants through individualized analysis of barriers and an action plan to navigate those barriers, including mental and behavioral health services;
  9. Responsible for updating a job board monthly;
  10. Accountable for assisting participants to develop résumés, navigate the job application process, and understand interview expectations;
  11. Promote programs through various outlets, including presentations, etc.;
  12. Responsible for answering telephone calls, responding to inquiries, and enrolling participants in desired programs;
  13. Responsible for providing excellent customer service; and
  14. Support additional tasks assigned by the Director of Programs.
Requirements & Qualifications
  1. Ability to function independently and to work effectively and in a collegial manner with co-workers and program participants.
  2. Effective oral and written communication skills.
  3. Possess certification or be willing to complete HIPAA training, Department of Justice Fingerprint Rolling Certification, and American Heart Association BLS (Basic Life Support) Provider and Instructor training programs within two months of employment.
  4. Must be a motivated self-starter and able to work effectively with limited supervision.
  5. Excellent organizational skills, including monitoring of details.
  6. Adhere to confidentiality mandates and professional conduct.
  7. Ability to effectively present information to different age groups and diverse populations.
  8. Proficient computer skills and in-depth knowledge of relevant software such as Word, Excel, PowerPoint, and Google Office, as well as a general understanding of database and reporting systems.
Preferred Skills
  1. Interest in direct services or case management.
  2. Experience working with diverse backgrounds; bilingual preferred (Haitian Kreole, Spanish, Arabic, French).
  3. Ability to create engaging materials and presentations.
Working Conditions, Job Setting, & Benefits
  1. Full-time - 40 hours/week.
  2. Work will be performed in community settings, remotely, and at NSDI headquarters in an office setting and requires electronic equipment, including a computer, telephone, etc.
  3. Flexibility in schedule, including scheduled weekends, when necessary.
  4. Visual/hearing ability sufficient to comprehend written/verbal and telephone communication. Ability to sit for long periods, stand, computer entry, walking, repeated bending, lifting and carrying up to 20 lbs, and reaching.
