The Staff Pad has partnered with Blue Ridge Medical Center to find a compassionate and experienced Registered Nurse Case Manager to join their team. This vital position ensures patients receive comprehensive, coordinated care that supports the best possible outcomes. The RN Case Manager plays a key role in patient advocacy, discharge planning, and care coordination across interdisciplinary teams.

What You'll Do:

  • Assess patient needs and develop individualized care plans from admission to discharge

  • Coordinate care and maintain communication between healthcare providers, patients, and families

  • Conduct utilization reviews, identify quality and safety concerns, and manage risk-related issues

  • Initiate and manage discharge planning, including follow-ups, referrals, and community resources

  • Advocate for patient access to necessary medical and support services

  • Maintain detailed documentation in electronic health records (EHR)

  • Participate in team meetings and case conferences to support continuity of care

Qualifications:

  • Active RN license in the state of Georgia (or Compact License)

  • Graduate of an accredited nursing program (BSN/MSN preferred)

  • Minimum 3 years of nursing experience

  • At least 2 years of case management experience required

  • BLS certification required

  • Preferred certifications: Certified Case Manager (CCM), Accredited Case Manager (ACM)

  • Strong communication, critical thinking, and organizational skills

  • Proficiency with Microsoft Office and EHR systems

Work Environment:

  • Clinical setting within a hospital environment

  • Occasional travel to various departments and offsite campuses

  • Exposure to infectious diseases possible; PPE provided

  • Physical activity includes standing, walking, bending, and occasional lifting
