Case Manager/Physical Therapist

Ensure total delivery of nursing care for residents in compliance with guidelines and regulations. Plan, provide, and direct resident care, complete assessments, transcribe MD orders, maintain Kardex, and assist in incident reporting. Prepare daily assignments, supervise Nursing Assistants, provide discipline and education, and participate in facility management. Receive occasional instruction from Nursing Supervisor and provide supervision to nurses aides, ward clerk, and volunteers. Work indoors with fluctuations in temperature, noise exposure, and potential outbursts of disruptive behavior. Requires frequent movement, lifting, climbing, stooping, standing/sitting, reaching, normal hearing and vision, and effective communication skills. Must have LPN license, 3-5 years of nursing experience, effective communication skills, problem-solving abilities, flexibility, and knowledge of nursing procedures and practices.
