Shift: M- F 8a- 5p
Pay: $29-$31 hourly
Contract Length: 13 weeks, 3/31-06/28
Duties:
- Collaborates with providers and clinic staff to identify patients with chronic diseases.
- Supports ongoing care for stable chronic conditions by assessing chronic conditions, reconciliating medications, providing education with approved materials, and referring to relevant agencies/resources.
- Assists in transitioning stable patients from inpatient to outpatient by assessing transitional needs, reconciliating medications, reviewing contingency plans, providing education, and assisting with post-discharge needs.
- Notify providers or RN of changes in patient's health
- Ensures accurate, timely documentation in the electronic record
- Prepare required reports for weekly meetings
- Maintains nursing competence and current evidence-based practices
- May perform clinical tasks within the scope of practice
- Performs other duties as assigned
Requirements:
- FL or compact state LPN license
- Current BLS Certification
- 2 or more years experience in Physician's office, Clinical, or Hospital setting.
- Knowledge of Chronic Diseases: Asthma/COPD, Diabetes, CHF and IHD.
- EMR experience
- Spanish/English Bilingual preferred.
