Job Title: Case Manager
Qualifications: A bachelor's degree in social work or related field or equivalent work/volunteer experience. Must work well with persons of varied racial, ethnic, cultural, socioeconomic backgrounds, and with individuals who are disabled. Demonstrated knowledge of crisis intervention and case management skills preferred. Previous employment or volunteer work with survivors of domestic violence or sexual assault preferred. Strong oral and written communication and computer skills are required. Bilingual (Spanish/English) speaker is a plus. Candidates must possess a reliable car.
Job Description:
- Conduct survivor intakes and service plans/assessments
- Provide clients with concrete and intensive case management and access to agency and external resources.
- Assist survivors in identifying and fulfilling their immediate/urgent needs.
- Consistently meet with survivors to develop and maintain relevant and meaningful service, safety, and stability plans.
- Educate survivors about dynamics of domestic violence and sexual assault. ? Refer survivors to other agencies and resources and advocate directly with other agencies as needed to help clients access necessary services.
- Maintain service partnerships with referral agencies.Manage incoming calls and walk-ins. Provide phone coverage during shifts, including intakes.
- Assess immediate crisis needs of victims during intake calls and perform appropriate actions.
- Visit with clients at safe house locations at least once per week and related field work, including but not limited to accompanying clients to court hearings, medical appointments, board of social services appointments, and safe housing placement and/or exit.
- Complete and maintain client records and document services provided to survivors within the agency database. Notes must be entered within 24 hours of the meeting (excluding weekends and holidays)
- Support the daily operations of the office, as assigned.
- Participate in agency meetings, case conferences, and assigned training. ? Perform other duties as assigned.
Schedule: Monday-Thursday in the office/field (9am-5pm with 1 day 11-7), Friday 9am-5pm remote.
Vehicle: Individual must have reliable personal transportation and will be asked to drive throughout shift to meet with their cases (mileage reimbursed).
Pay: $20-25/hr
