Key Responsibilities: - Conduct thorough assessments to identify the needs of individuals experiencing homelessness.
- Develop and implement personalized case management plans to address housing, employment, healthcare, and other essential services.
- Provide ongoing support and advocacy to clients, ensuring they have access to necessary resources and services.
- Collaborate with local agencies, shelters, and service providers to coordinate care and support for clients.
- Maintain accurate and up-to-date client records and documentation.
- Monitor and evaluate client progress, adjusting case management plans as needed.
- Participate in community outreach and education efforts to raise awareness about homelessness and available resources.
Qualifications: - Bachelor's degree in Social Work, Psychology, or a related field.
- Previous experience in case management, social services, or working with homeless populations is preferred.
- Strong understanding of the challenges faced by individuals experiencing homelessness.
- Excellent communication, organizational, and problem-solving skills.
- Ability to work independently and as part of a team.
- Compassionate and empathetic approach to client care.
- Proficiency in Microsoft Office and case management software.
