Case Manager - Philadelphia, PA 19144
LHH Recruitment Solutions is partnering with a local organization to hire a Case Manager to support residents in finding stable housing and achieving their personal goals. This is a full-time, on-site role with a trauma-informed approach to case management.

Key Responsibilities:
  • Manage a caseload of 15 or more residents at any given time.
  • Develop individualized plans for residents upon their arrival at the shelter.
  • Provide ongoing support and guidance to ensure residents stay on track with their housing and personal development goals.
  • Conduct regular meetings with residents to assess progress and adjust plans as needed.
  • Utilize internal system todocument case notes and track resident progress.
  • Work closely with the resident psychiatrist and other support staff to address mental health and well-being.
  • Navigate high-stress situations and effectively use de-escalation techniques when necessary.
Schedule & Compensation:
  • Monday - Friday, 8 AM - 4 PM or 9 AM - 5 PM (some flexibility available).
  • 40 hours per week with a paid lunch break.
  • 100% on-site position.
Qualifications:
  • Bachelor's degree required (social work preferred but not required with relevant experience).
  • Previous case management experience is essential.
  • Experience working with young people in underserved communities.
  • Ability to thrive in a high-stress environment and effectively manage crises.
  • Strong interpersonal and communication skills, with a comfort in de-escalation techniques.
