Case Manager

Seeking an enthusiastic Licensed Practical Nurse (LPN) or Certified/Registered Medical Assistant (MA) for a temporary .8 FTE, 6-month oncology position, days in clinic nursing.
•  Work closely with providers to deliver quality patient care, maintain patient flow, administer medications, update records, collect/verify info, and manage exam rooms.
•  Responsibilities include focused patient assessments, implementing care plans, delegating/assigning tasks, patient advocacy, education, and policy input.
•  Requires current LPN license or MA certification, BLS (within 2 months), and strong interpersonal and organizational skills.
•  Ambulatory care experience preferred. EEO/AA Employer.
