We are seeking Home Health RN Case Managers to join our dedicated team providing compassionate home health care in central Maryland. This role involves delivering individualized nursing care in patients' homes, coordinating services, and managing clinical resources from pre-admission to post-discharge.
Responsibilities:
Perform primary nursing care functions under clinical manager's direction. Coordinate care with multiple clinicians and community resources. Manage patient transition planning and clinical resources. Qualifications:
RN licensure in Maryland required. Prior hospice or home health experience preferred. Valid driver s license and reliable transportation. Compensation: $38.05 - $57.05 per hour, with benefits including PTO, tuition reimbursement, and health coverage.
