LHH Recruitment Solutions is seeking an organized and compassionate Case Manager to join our team at Highland Gardens in Los Angeles. The Case Manager will provide support and guidance to residents by connecting them to community resources, ensuring they achieve stability, and fostering a positive living environment. This role requires a proactive and empathetic individual who is passionate about helping individuals overcome challenges and reach their full potential.

Key Responsibilities:
  • Conduct needs assessments for residents and develop tailored action plans.
  • Coordinate access to community resources, including housing, healthcare, and employment services.
  • Serve as the primary point of contact for residents, advocating on their behalf and providing ongoing support.
  • Maintain detailed and confidential records of resident interactions and progress.
  • Collaborate with external agencies, service providers, and internal teams to ensure comprehensive resident support.
  • Monitor and evaluate the effectiveness of case plans, making adjustments as needed.
  • Facilitate workshops or informational sessions for residents to build life skills.
  • Respond promptly to resident concerns and emergencies, maintaining professionalism under pressure.
Qualifications:
  • Bachelor's degree in Social Work, Psychology, Human Services, or a related field (equivalent experience may be considered).
  • Prior experience in case management, social services, or a similar role.
  • Strong understanding of local community resources and social support systems in Los Angeles.

Pay Details: $23.00 to $26.00 per hour
