A nonprofit services organization in New York City is looking to add a new Case Manager to their staff in the Bronx. In this role, the Case Manager will be responsible for providing case management services to formerly homeless individuals, some with multiple disabilities, in order to help the individual transition and adjust to permanent housing.
About the Opportunity:
- Ideal Start Date: ASAP
- Schedule: Monday to Friday
- Hours: 9am to 5pm
- Caseload: 13 to 16 patients
Responsibilities:
The Case Manager will:
- Oversee case coordination for a caseload of 15, including home visits in clients apartments; provide escort to appointments, as needed
- Conduct an initial and ongoing assessment of clients , including completion of biannual assessments and psychosocial assessments
- Provide crisis intervention, when needed
- Connect clients to an array of services as appropriate
- Maintain record keeping in compliance with agency standards as well as those of City and State agencies
- Support group facilitation, including planning and participating in events, such as holiday parties, annual BBQ, etc.
- Perform other related duties as assigned
Qualifications:
- 2 years of experience working with people with co-occurring disorders
- Proficiency with Microsoft Office suite and databases
- Knowledge of issues of the Homeless population
- Knowledge of Social Services systems
- Microsoft Office Suite proficient
- Solid assessment and documentation skills
- Great interpersonal skills
- Excellent communication skills (written and verbal)
- Strong attention to detail
- Highly organized
Desired Skills:
- Bilingual (English and Spanish)
- Bachelors Degree
- Training in Harm Reduction, Housing First, Motivational Interviewing, and Crisis Intervention
