Pay: Uncertified Case Managers start at $17.50 - Uncertified Case Managers will obtain certification within the first 90 days and pay will be increased
Certified Case Manager range starts at $18.76 - Certified Case Manager pay is calculated based on years of certified experience

Schedule: Mon-Fri, 8am-5pm

Job Summary & Deliverables

Valley Oaks is a group home for men specializing in maintaining mental health stability and sobriety. Case Managers will be responsible for passing medications, passing food at meal times, prompting independence, and encouraging healthy habits.

Case Managers provide psychosocial rehabilitative services as indicated by the client's care plan to support clients in meeting their individualized goals. Case Managers are responsible for utilizing evidence-based practices, meeting fiscal responsibilities, and ensuring that regulatory compliance and organizational standards are met.
  • Provides psychosocial rehabilitative services as described by the Medicaid manual
  • Supports clients in following individualized care plans and identifying case management goals
  • Completes documentation on services provided within Valley's standards
  • Meets productivity expectations as assigned by supervisor
  • Distribute medications as necessary
  • Transports clients to appointments as necessary
Requirements
  • High School diploma or equivalent
  • Case Manager certification must be obtained within 90 days Valley assists with certification
  • CPR certification Valley provides as needed
  • Driving positions require a current Driver's License, must be 21
Preferred Qualifications
  • Previous behavioral health experience
  • Bachelor's degree in social services, Psychology, or related field

Salary Description

$17.50/hr rate increases to $18.76w/CM certificate
