We are seeking a dedicated and proactive Wellness Care Coordinator to join our team and play a critical role in supporting employees' health and well-being without requiring them to leave the workplace. This position is ideal for a compassionate healthcare professional who excels in care coordination, chronic condition management, and wellness coaching.
Key Responsibilities:
- Wellness Coaching: Provide 1:1 coaching and education to employees to support lifestyle changes and chronic condition management (e.g., hypertension, diabetes).
- Chronic and Acute Care Support: Offer information and guidance regarding chronic health conditions and assist with acute care needs in collaboration with Primary Care Providers (PCPs).
- Care Coordination:
- Work directly with patients and PCPs to manage medications, adjust treatments, and schedule appointments.
- Assist in finding new doctors and facilitate referrals when necessary.
- Monitor blood pressures and coordinate follow-up care and medication adjustments with PCPs.
- Care Gap Closure: Identify and address care gaps using employee health data to ensure timely preventive care and follow-ups.
- ED Utilization & Inpatient Stay Reduction: Monitor and help reduce unnecessary emergency department visits and hospitalizations through proactive engagement and intervention.
- Data-Driven Outreach: Utilize the SpringBuck platform to track key metrics such as care gaps, ED utilization, spend of care, and inpatient stays to inform outreach strategies.
- Onsite Convenience: Enable employees to receive necessary care and coaching services at the workplace, enhancing accessibility and minimizing disruption to their schedules.
Qualifications:
- RN Certification
- Experience with care coordination, patient education, and wellness coaching
- Strong communication and interpersonal skills
- Comfortable working with healthcare data platforms (experience with SpringBuck is a plus)
- Ability to work collaboratively with PCPs, specialists, and internal wellness teams
- Passion for improving health outcomes and closing care gaps in a workplace setting
