Community healthcare facility looking to bring on Director Case Management. Bonus Incentive Plan.
Responsible and accountable for clinical and financial operations of the Care Management system. The Director provides leadership through effective managerial and educational direction in the areas of Care Management operations, resource management, patient flow through the continuum.
- Position reports to CEO.
- Open to licensed RN or SW/MSW.
- Director CM does not carry a patient case load.
- Direct reports - currently 8 (3 CM fulltime, 5 CM PRN) mix of RN/SW.
- Must be willing to acquire Certification in Case Management within 1st year in position if not already a Certified Case Manager.
- 2+ years of acute care hospital leadership in case management experience, very professional and strong customer service.
- 38 average daily patient census.
