Care Manager Requirements
Bachelor's degree in a related field preferred (required for detox level of care); high school diploma required at minimum
Minimum of one year of experience working in the substance use disorder field
Knowledge of addiction, mental health, treatment resources, and 12-Step recovery
Familiarity with discharge planning, aftercare coordination, and case management
CPR certification preferred
CRSS, CAC, CAP, LCSW, LMHC, or Registered Intern (preferred, willingness to obtain a plus)

Care Manager Responsibilities
Coordinate discharge planning and assist clients with transitions to aftercare programs
Communicate with outside providers, referents, and family members (with proper releases in place)
Facilitate individual case management sessions and maintain ongoing communication with clients
Participate in intervention strategies for clients at risk of leaving treatment Against Medical Advice (AMA)
Monitor client participation, attendance, and adherence to the daily treatment schedule
Assist with clinical documentation, including assessments and treatment planning, as appropriate
Facilitate weekly clinical groups and participate in recreational and weekend activities as needed
Maintain accurate and timely documentation in accordance with company standards
Coordinate and assist with client transportation as needed
Assist clients with daily living essentials and ancillary financial accounts
Monitor and support agency drug screening processes under supervision
Uphold patient privacy and confidentiality in accordance with HIPAA and 42 CFR regulations
Report concerns about client privacy, safety, or quality of care to appropriate leadership without fear of retaliation
Support and enforce facility policies, procedures, and program expectations
Perform other duties as assigned

Pay: $40,000-60,000
