Daybreak Ministries Care Coordinator Description

Screen and Intake prospective clients to enter the program with proper forms
Refer new clients for medical services to established pregnancy centers for pregnancy testing, ultrasound, and STD testing
Assist in obtaining government assistance such as WIC, etc.
Assist the Executive Director in providing babies' essential needs
Coordinate activities in support of clients, including prenatal education and
parenting education
Determine housing needs, such as rental assistance, and refer to the Executive Director
Provide the guidelines, policies, and procedures for clients and gain their signed agreements
Explain to clients opportunities for continued education or employment and guide them in the process
Explain to clients Daybreak Ministries' adoption referral policy
