Job Title: Senior Care Coordinator


Location: Burnley, UK


Employment Type: Permanent


Salary: 28,000 per annum

Responsibilities:


  • Coordinate and manage care services for clients, ensuring their individual needs and preferences are met
  • Develop and maintain care plans for clients, reviewing and updating as necessary
  • Supervise and support a team of care workers, providing guidance and training as needed
  • Conduct assessments and risk assessments for new clients and review existing clients' care needs
  • Liaise with healthcare professionals, families, and other stakeholders to ensure effective communication and coordination of care
  • Monitor and maintain accurate records and documentation in accordance with company policies and regulatory requirements
  • Participate in on-call duties on a rotational basis to ensure 24/7 support for clients
  • Identify and address any issues or concerns raised by clients or their families in a timely and professional manner
  • Contribute to the development and implementation of policies and procedures to improve the quality of care services provided
  • Support the recruitment and onboarding of new care workers, conducting interviews and completing necessary checks and documentation
  • Requirements:


  • Minimum of 2 years' experience in a care coordination or similar role in a domiciliary care setting
  • NVQ Level 3 in Health and Social Care or equivalent qualification
  • Knowledge of complex care needs and experience in managing care for clients with complex conditions
  • Excellent communication and interpersonal skills, with the ability to build positive relationships with clients, families, and healthcare professionals
  • Strong leadership and supervisory skills, with the ability to motivate and support a team
  • Sound understanding of relevant legislation and regulatory requirements in the care sector
  • Proficient in Microsoft Office and electronic care management systems
  • Ability to work flexible hours and participate in on-call duties as required
