Responsibilities:
Coordinate and manage care services for clients, ensuring their individual needs and preferences are metDevelop and maintain care plans for clients, reviewing and updating as necessarySupervise and support a team of care workers, providing guidance and training as neededConduct assessments and risk assessments for new clients and review existing clients' care needsLiaise with healthcare professionals, families, and other stakeholders to ensure effective communication and coordination of careMonitor and maintain accurate records and documentation in accordance with company policies and regulatory requirementsParticipate in on-call duties on a rotational basis to ensure 24/7 support for clientsIdentify and address any issues or concerns raised by clients or their families in a timely and professional mannerContribute to the development and implementation of policies and procedures to improve the quality of care services providedSupport the recruitment and onboarding of new care workers, conducting interviews and completing necessary checks and documentationRequirements:
Minimum of 2 years' experience in a care coordination or similar role in a domiciliary care settingNVQ Level 3 in Health and Social Care or equivalent qualificationKnowledge of complex care needs and experience in managing care for clients with complex conditionsExcellent communication and interpersonal skills, with the ability to build positive relationships with clients, families, and healthcare professionalsStrong leadership and supervisory skills, with the ability to motivate and support a teamSound understanding of relevant legislation and regulatory requirements in the care sectorProficient in Microsoft Office and electronic care management systemsAbility to work flexible hours and participate in on-call duties as requiredIf you are a dedicated and experienced Care Coordinator looking for a new challenge and the opportunity to make a real difference in people's lives, we would love to hear from you.
