Care Coordinator Highlights
- The pay for this position is based on experience.
- Must be available to work any 8-hour shift between 7:00 AM and 7:00 PM CST.
- 100% Remote position.
- This position is a full-time, contract role.
Responsibilities
- Apply acquired job skills and company policies to complete assigned tasks.
- Work on routine assignments that require basic problem resolution and guidance from policy and past practices.
- Enter accurate referrals and applications into the customer relationship management platform.
- Resolve patient questions regarding the status of their therapy or journey.
- Document accurate, detailed notes in proprietary software while on patient calls.
- Make outbound calls for follow-up or confirmations.
- Maintain quality and empathy while educating patients during conversations.
- Interact with referral sources, including healthcare providers, to process applications.
- Manage patient accounts from first contact to final approval or denial.
Requirements
- High School Diploma or equivalent required.
- 2-4 years of customer-facing experience preferred, especially in call center or customer service roles.
- Strong communication skills-verbal and written-with a patient-focused and empathetic approach.
- Ability to type at least 40 WPM and transcribe notes during calls.
- Proficient with Microsoft Office and video conferencing tools (Zoom, Microsoft Teams).
- Independent and self-motivated with strong time management skills and attention to detail.
- Familiarity with insurance billing or pharmacy operations is a plus.

- Identify progress toward desired care outcomes; intervening to overcome deviations in the expected plan of care; reviewing the care plan with patients in conjunction with the direct care
- Identify the primary care team involved in the specialty patient care
- Utilize assessment skills and risk assessment tools to identify patients with actual or potential care needs that would require care coordination
- Promote adherence to a care plan
- Connect patient back to primary care physician and primary care coordinator team
