A high level of clinical decision making is required for this role and the post holder will be expected to develop, demonstrating your ability to make clinical decisions autonomously supported by senior cardiology team.

You will need to work proactively to deliver a responsive service to patient with both planned and unplanned care needs.

The post holder will work collaboratively with other colleagues to provide patients with a holistic needs assessment, offering education to achieve a person centred approach, promoting a culture which enables patients to focus on self-management.

Main duties of the job
  • Provide clinical and professional leadership to nursing staff
  • Follow up the patients with chest pain, acute or chronic cardiac conditions who are not admitted to cardiology wards to ensure that evidence based treatment is applied and drug therapies are adhered to.
  • Provide advice and guidance in the management of patients referred and transferred from District General Hospitals
  • Identify and discuss with the on-call cardiologist patients who require referral to cardiology
  • Fast track referrals for investigations as required, to ensure timely treatment and potentially reduce length of hospital stay
  • Risk assesses all patients and provides lifestyle advice and literature
  • Ensure that clinical practice and the development of clinical guidelines adhere to guidelines produced by regulatory bodies
  • Liaise with ECC, CCU, medical, cardiology wards and primary care teams as appropriate to the needs of the service
  • Provide clinical and professional leadership
  • Provide nurse led rapid access chest pain clinic
  • Support and undertake diagnostics including Rapid Access Chest Pain Clinic, Dubutamine Stress Echo's and other diagnostics as required.

Detailed job description and main responsibilities

Planning and Organisational Duties

  • Support cardiology nursing colleagues by providing cross cover for annual leave and other leave as necessary
  • Support clinical activity as part of the weekly job plan.
  • Support diagnostic actively through the week and flex duties to support this activity.
  • Support research based practice
  • Provide teaching and education sessions for nurse development

Communications and Key Working Relationships

  • Work with NICE guidelines and communicate best practice to wards and departments caring for cardiac patients.
  • Support the cardiac network working with the CATs system

Responsibility for Health & Safety

  • Compliance with the Health & Safety at Work Act 1974 - the post holder is required to fulfil a proactive role towards the management of risk in all of their actions. This entails the risk assessment of all situations, the taking of appropriate actions and reporting of all incidents, near misses and hazards, and a statutory duty of care for their own personal safety and that of others who may be affected by their acts or omissions.

Person specification Qualifications
  • Registered Nurse
  • Degree (or working towards)
  • Post graduate Certificate in Cardiac Care
Experience
  • Able to demonstrate experience at Band 6 or equivalent level in Cardiology
  • Evidence of research/audit experience and presentations skills to large audiences of staff
  • Experience of acute and chronic Cardiology conditions and treatment pathways
Skills
  • Display initiative

  • Enthusiastic, confident with a Page 2 of 4 positive attitude
