What you bring to SBHG (Qualifications):

A master's degree in social science is required, plus one (1) year of experience working in behavioral health.

Or-

A Bachelor's degree in social science is required, plus (2) years of experience working in a behavioral health field.

Or-

Associate's degree in social science required, plus (3) years of experience working in a behavioral health field.
Valid California Driver's License
Meets Agency driver eligibility criteria

This position coordinates the care and serves as ECM Point of Contact/Lead Care Manager for the Full Circle Health ECM qualifying youth.

Division/Program Overview:

An ECM Manager is a Lead Care Manager specializing in Enhanced Care Management, providing comprehensive support to enrolled individuals. They act as a central point of coordination, connecting members with vital resources such as housing, food, and childcare, while also collaborating with diverse healthcare providers to ensure seamless and coordinated care.
