bilingual in Spanish Overview: LHH Recruitment Solutions is seeking a
Case Manager to support permanent supportive housing residents, primarily formerly homeless seniors. The role focuses on rapid placement into housing and immediate case management support to help residents integrate into the community.
Key Responsibilities: - Work with new residents immediately upon referral to facilitate their transition and acclimation to the community
- Conduct regular case management meetings to assess needs and provide linkages to services (e.g., primary care, mental health, benefits assistance)
- Adjust case management frequency based on residents' stability and level of support needed
- Organize and coordinate community events for seniors, ensuring participation and engagement
- Assist with workflow related to events, meeting with clients, taking notes, and documenting
- Collaborate with other case managers to ensure clients receive the appropriate support
- Support residents dealing with physical, mental, and behavioral health challenges, including substance abuse issues
- Maintain a caseload of 10-12 residents, ensuring personalized care and assistance
Preferred Experience & Skills: - Experience working with seniors (62+), diverse populations, and individuals with complex needs
- Background in permanent supportive housing, case management, or social services
- Ability to support individuals with physical, mental, and behavioral health concerns
- Fluency in Spanish is required to effectively assist a predominantly Spanish-speaking caseload

Pay Details: $23.00 to $28.00 per hour
