Behavioral Health Care Coordinator

Position Summary

The Behavioral Health Care Coordinator (BHCC) position works closely with the Behavioral Health Consultation team and Access Medical Providers to support care coordination efforts for our patients struggling with mental health and substance use disorders. BHCCs assist patients, both in-person and by phone, with connection to specialty mental health and substance use disorder services. BHCCs communicate and advocate with community agencies to support safe and effective patient care. BHCCs maintain updated knowledge and information about community agencies/resources to support our patients in accessing the services they need. The purpose of this position is to provide enhanced support to specific patient populations within Access to improve patient outcomes and support optimal care. Team communication and teamwork are highly valued in this role.

The Behavioral Health Care Coordinator position will be housed within the Behavioral Health team, managed by the Primary Care Behavioral Health Manager under the Chief Behavioral Health Officer. This role is focused on supporting: 1) patients on Medication Assisted Treatment (MAT); 2) patients with Severe and Persistent Mental Illness (SPMI); and 3) patients with uncontrolled diabetes (a19) with comorbid mental health diagnosis that interferes with effective self-management. These groups have been selected to align with existing clinic initiatives and to assist in filling an identified gap in current services. Desired candidate would have the following qualifications:

Required Qualifications:

  • Masters Degree in Social Work
  • Strong teamworking skills and desire to work as part of a team
  • Strong communication skills, both written and verbal
  • Passion for working with a diverse patient population
  • Strong organizational and time management skills
  • Ability to use or learn to use Epic
  • Ability to maintain confidentiality of protected health information

Desired Qualifications:

  • Knowledge of community resources/agencies in Dane County
  • Comfort working with people struggling with mental health and substance use concerns
