ASW Assistant Team Manager/Practice Manager, Southwark £40.69ph

Location: Southwark

Working Hours: Tuesday to Saturday (36 hours/week)

Role Overview: Lead with Purpose as Deputy Team Manager in Southwark's Hospital Discharge Transfers of Care. This leadership role involves overseeing hospital discharges during weekends, ensuring smooth patient transitions and effective team management.

About the Service:

The Hospital Discharge Transfers of Care team focuses on seamless care transitions from hospital to home or other settings, managing complex discharges, safeguarding, and care planning with compassion.

Key Responsibilities:
  1. Operational Leadership: Manage daily operations from Tuesday to Saturday, ensuring efficient triage, case allocation, and assessments.
  2. Guidance and Support: Provide expert advice on complex discharges, safeguarding, and mental capacity assessments.
  3. Collaborative Networking: Lead weekend discharges at Kings and St Thomas Hospitals, coordinate with extra care schemes for resident placements.
  4. Team Development: Mentor and supervise social workers and assistant practitioners to promote continuous learning.
Candidate Profile:
  • Strong knowledge of the Care Act, Mental Capacity Act, and Continuing Healthcare.
  • Experience leading MDT and safeguarding meetings, providing reflective supervision.
  • Proven leadership skills with confidence in decision-making.
  • Supervises 4-5 qualified social workers.

Note: The role requires presence at St Thomas and Kings College Hospitals, with approximately 4 days/week onsite and up to 1 day/week remote work for administrative tasks.
