Job Details:
- BC/BE FM or IM
- M-F 8AM-5 PM
- Call is 1:7
- 13-16 pts per day
- Chronic illnesses with substance abuse Candidate must be OK with prescribing opioids
- Complex care, Incoming patients are invite-only
- Looking for someone who has experience treating patients with multiple chronic diseases and truly believes in the value of a team-oriented patient-centered medical home model
- EMR is E clinical works, Onsite infusion center
- BLS certification and a strong focus on customer service is required
- Experience in population health management and working with psychosocially and medically complex patients is strongly preferred
