Description
This is a Care Management Supervisor II position within the Area Agency on Aging for Luzerne/Wyoming Counties. The employee in this position is expected to manage the Care Management Unit and carries over-all responsibility for the work of the unit. In addition to supervising the Care Management Supervisor I's, Care Manager II's, Clerical and case aides in the Care Management Unit, this employee also performs administrative activities related to the Caregiver Support, Dom Care, Cost Sharing and Consumer Reimbursement program. In addition this employee acts as a liaison with providers of guardianship or representative payee cases.

Essential Duties and Responsibilities

Ensuring the policies and procedures of the Pennsylvania Department of Aging's Option services, Cost Sharing and Care Management programs are followed. Monitoring and implementing any new revised policies or procedures directed by the State to remain in compliance with all regulations. Attend all Webinars and telecommunication calls from the State Department of Aging to learn and implement any policy/procedure changes.
Administration of the Caregiver Support, Consumer Reimbursement and Domiciliary Care programs which includes the following activities:
a.) Maintain ongoing materials and a community resource guide for caregivers.
b.) Maintain ongoing assessments/reassessments of caregivers needs.
c.) Monitoring system of caregiver reimbursement in the Caregiver Program and
Consumer Reimbursement Program.
d.) Overseeing the system of provider certification and care management of consumers in the Domiciliary Care Program

Liaison with providers of the Representative Payee, guardianship and other financial services transition services includes:
a.) Arranging for evaluations of individuals where guardianship is being requested, arranging for guardianship petitions to be filed when appropriate, monitoring ongoing guardianship services provided by contractor for that service.
b.) Monitoring of Representative Payee services: arranging for Legal Services for the handling of small estates.

• Representing care management staff's needs and concerns to administrative staff to ensure good working relationships within the agency;

• Planning, organizing and assigning initial cases to care management staff to ensure timely completion of assessments. Tracking assignments to identify and address problems;

• Reviewing initial assessments and approving services after case conference with worker.

• Reviewing the NAT/LCD, service orders, care plans, worksheets, financial worksheets, cost calculation sheets, journals, details section in SAMS/Harmony for accuracy;

• Enter State required data into SAMS on case reviews;

• Review Cost Cap Exception cases for accuracy to submit to the State for final approval;

• Responsible for LCD certification process of all CM Supervisor I's;

• Ascertain the training needs of care management staff and arranging for appropriate training programs; providing information to staff on resources available to the aging population;

• Maintain a monthly excel report tracking all Home Modifications, Chore, Assistive devices, and Special Funding for Personal care/respite. Forward copies of the excel Special Funding reports to Executive Director, Social Service Director, Fiscal Department, MIS and Contract Monitor monthly;

• Complete reports as directed by administration;

• Complete yearly performance evaluations of Supervisor I employees and clerical staff assigned to the unit;

• Evaluating staff performance and making recommendations regarding personnel actions;

• On a regular basis make supervisory field visits;

• As directed by supervisor, make presentations to community groups explaining Aging Services;

• On a rotation basis screen after hour crisis calls; supervise care manager on call;

• Serve on aging committees as directed by supervisor;

• Participate in review process of invoices from service providers and caregivers;

• Knowledge of casework principles and methods.

• Knowledge of current social, economic and health problems and community resources as related to older adults.

• Knowledge of individual and group behavior and ways of working effectively with older adults who have social, economic, emotional and/or health problems.

• Knowledge of the basic principles and methods of program interpretation and community organization.

• Knowledge of basic principles and practices of supervision.

• Skill in the development of plans to address the needs of vulnerable older adults.

• Ability to work effectively with older adults and to aid them to grow in the constructive use of their potential in adjusting to their specific problems.

• Ability to evaluate work of subordinates and to provide technical assistance and guidance.

• Ability to orient and instruct subordinates on procedures, techniques, laws and regulations.

• Ability to establish and maintain effective working relationships with clients, other members of the staff, outside agencies and institutions, and the general public.

• Ability to clearly express ideas orally and in writing.

ELIGIBILITY- ALL CANDIDATE S):
1.) Meet the minimum experience and training (MET'S) required for this position:
Four years of experience in public or private social work; and a Bachelor's Degree; or any equivalent combination of experience and training including successful completion of 12 semester hours of college level courses in sociology, social welfare, psychology, gerontology, or other related social sciences.
2.) Be a resident of Pennsylvania.
3.) State Civil Service Commission Approved Additional Special Requirements: None
4.) Be eligible for selection in accordance with merit system employment regulations

  • The absence of any discipline above the level of written reprimand during the 12 months preceding the closing date on the posting.
  • The last due overall regular or probationary performance evaluation was higher than unsatisfactory
