Our Frailty Service has supported Brownlow's housebound population to achieve better health outcomes and wellbeing. We are expanding our dedicated team and seeking a passionate professional to join us.

Role Overview:

As a key member of the Brownlow Health Frailty Team, you will develop a new Acute Home Visiting Service, complementing our existing services. This innovative service will be supported by a multidisciplinary team.

Main Duties

Key responsibilities include:

  • Performing acute and planned home visits for patients within the Brownlow Health housebound population, including those who are frail, living with dementia, homeless, refugees, or asylum seekers.
  • Conducting holistic assessments, managing acute health issues and long-term conditions, and referring patients to appropriate services while collaborating with patients, families, and external providers to develop care plans.

Job Responsibilities

As an Advanced Nurse Practitioner, you will work within a nurse-led multidisciplinary team, providing both acute and proactive planned home visits for housebound patients.

You should demonstrate sound knowledge of advanced nursing practice, be an experienced non-medical nurse prescriber, and work autonomously.

This role offers an exciting opportunity to support the development of an acute home visiting service within an established framework.

Clinical Responsibilities
  • Providing acute and planned home visits for frail, dementia-affected, homeless, refugee, or asylum-seeking patients.
  • Managing long-term conditions proactively.
  • Conducting holistic assessments using the British Comprehensive Geriatric Assessment Tool.
  • Managing acute illnesses, including undifferentiated conditions and red flag presentations.
  • Using non-medical prescribing skills to initiate and manage treatments.
  • Screening for dementia, depression, and other conditions, ensuring timely referrals.
  • Reviewing medications and optimizing prescribing, mindful of anti-cholinergic burden.
  • Delivering immunisations during home visits to reduce follow-ups.
  • Facilitating care planning, including anticipatory care, DNACPR decisions, and GSF registration.
Collaborative and Holistic Care
  • Working with GPs, geriatricians, mental health teams, community nurses, social workers, and others.
  • Supporting informal carers and liaising with external agencies.
  • Engaging in safeguarding and MDT reviews for vulnerable patients.
Leadership and Development
  • Leading the development of the acute home visiting service.
  • Mentoring junior staff and contributing to educational updates.
  • Leading or participating in clinical audits and quality assurance.
  • Promoting clinical excellence and continuous learning within the team.
Research and Education
  • Supporting research projects and educational initiatives.
  • Collaborating with academic and public health bodies to embed evidence-based practices.
  • Presenting audits and case studies and contributing to policy development.
Other Responsibilities
  • Maintaining confidentiality and adhering to codes of conduct.
  • Complying with health and safety regulations, reporting hazards, accidents, and incidents.
  • Promoting safeguarding of children and vulnerable adults, attending relevant training.
