TheAdvanced Nurse Practitioner(ANP) has been assessed as competent inpractice using their expert clinical knowledge and skills. They have thefreedom and authority to act and make autonomous decisions in the assessment,diagnosis and treatment of patients.
TheANP provides an opportunity for patients to avoid the need to be referred to aGP, thereby enabling them to receive timely care and negate unnecessary delayin receiving treatment.
ANPsin primary care can develop close, long-term relationships with their patientsand work in partnership with them to achieve optimum health. They areautonomous in making decisions based on assessment, diagnosis andinterpretation of test results. They can independently prescribe appropriatemedication, evaluate or refer to other specialists if necessary.
Main duties of the job
Responsibilities of the Advanced Nurse Practitioner. (not exhaustive).
Implement health promotion and wellbeing programmes
Manage patients presenting with acute or chronic medical conditions
Evaluate individual specialised treatment plans for chronic disease patients
Identify and manage patients at risk of developing long- term conditions
Provide advanced care to patients as required in accordance with clinical based evidence
Request, process and interpret pathology services and other tests as necessary
Deliver patient care as necessary, referring patients to secondary/specialist care as required
Maintain accurate clinical records in conjunction with legislation
Ensure coding is used effectively
Maintain chronic disease register
Prioritise health issues and intervene appropriately
Support the team with clinical emergencies
Recognise, assess, refer patients presenting with mental health needs
Be an extended and supplementary prescriber, adhering to extant guidance
Support patients in the use of their prescribed medicines (within own scope of practice), reviewing annually as required
Contribute to practice targets (QOF, etc.), complying with local and regional guidance
Liaise with external services/agencies to ensure the patient is supported appropriately
Delegate clinical responsibilities appropriately, ensuring safe practice (within the scope of practice)
Support the clinical team with all safeguarding matters in accordance with policy
Deliver opportunistic health promotion where appropriate

Job responsibilities
In addition to the primary responsibilities alreadydetailed, the Advanced Nurse Practitioner may be requested to:
Act as the audit lead,effectively utilising the audit cycle
Act as the IPC lead for thepractice, ensuring compliance, undertaking audit as necessary
Monitor and ensure the safestorage, rotation and disposal of medicaments
Undertake overall management ofthe nursing team, providing guidance when necessary, acting as a mentor tostudents and newly qualified staff
Maintain an effective nursingstaff rota ensuring all clinics are staffed appropriately
Participate in local initiativesto enhance service delivery and patient care
Support and participate in sharedlearning within the organisation
Develop an area of specialistinterest, taking the lead within the organisation
Continually review clinicalpractices, responding to national policies and initiatives where appropriate
Participate in the review ofsignificant and near-miss events applying a structured approach i.e., rootcause analysis (RCA)
Drive the development of nursingservices within the organisation, liaising with external agencies andprofessional organisations as required
Develop practice administrative and clinical protocols inline with the needs of the patient and current legislation
Person Specification Qualifications
- Registered Nurse with Nursing and Midwifery Council
- Masters degree required for qualification or
- Post graduate diploma or degree for Advanced Practice Qualification
- Qualified Independent Nurse Prescriber on the NMC register
- Meets the standards for registered ANP working at advanced level
- Qualified triage nurse
- Teaching qualification
- ALS and PALS
Experience
- Experience of practice within the four pillars
- Job plan that demonstrates advanced nursing practice and has equity with peers working at this level
- Experience of prescribing and undertaking medication reviews
- Experience of working in a primary care environment
- Experience of working as a practice nurse or community nurse
