Advanced Nurse Practitioner/ Advanced ECP - Home Visiting Clinician

Working as part of the Care Home Enhance Rapid Response service (CHERRS)

The aim of this service is to target urgent, emergency, unscheduled, and acute episodes of care in patients residing in care homes who would otherwise be at risk of hospital attendance or admission. The service provides a rapid response that ensures the unwell patient receives prompt examination and treatment by an advanced practitioner. You will be expected to assess episodes of acute/chronic illness and will be competent in clinical examination, assessment, diagnosis, and treatment, including prescribing where appropriate.

Main duties of the job

Must have Prescribing Qualification

To succeed, you will be a practicing advanced nurse practitioner/advanced paramedic with previous experience in urgent and emergency care. You will demonstrate a high standard of clinical skills and have the ability to communicate, liaise, and interact effectively with multi-professional teams across organizational boundaries, including ambulance services, primary care, social services, and secondary care providers.

Strong organization and time management skills are essential, as is the ability to prioritize workload. Experience in using various clinical IT systems and basic IT literacy is required.

About us

The ELM Alliance is a not-for-profit federation of all independent GP practices throughout South Tees, covering over 300,000 patients. Members remain independent organizations but collaborate on developing local primary care.

The ELM Alliance CHESS service was first commissioned in June 2017 and has secured ongoing funding since June 2018. The service operates Monday to Sunday, 8 am - 6 pm, excluding Bank Holidays.

Job responsibilities

KEY RESPONSIBILITIES

Clinical

  • Provide assessment at first contact by attending to patients in care/nursing homes with acute/chronic conditions.
  • Prioritize patients, including urgent cases, demonstrating clinical assessment skills to obtain history, examine, diagnose, and treat, including non-medical prescribing where appropriate.
  • Evaluate clinical information and initiate appropriate treatment or referral.
  • Undertake and interpret diagnostics and take appropriate actions to manage patient care.
  • Signpost to other services within healthcare pathways.
  • Refer patients to appropriate care settings within local pathways.
  • Discharge or treat patients as appropriate within scope of practice.
  • Prescribe as an independent non-medical prescriber.
  • Maintain accurate documentation of patient interactions and clinical care, including adverse events.
  • Provide clinical advice to team members where appropriate.

    Maintain patient and staff confidentiality under the Data Protection Act 1998. Do not disclose sensitive information unless in the recognized course of duty. Seek advice from your line manager if uncertain.

    • Team cooperative, professional development-oriented, and initiative-taking.
    Experience
    • Workload management, autonomous practice, urgent/emergency care, care home experience, and IT skills.
    Other requirements
    • Vaccination status, ability to handle sensitive issues, flexibility, and a valid driver's license. Enhanced DBS required.
    Qualifications
    • Registered Nurse (NMC) or Paramedic (HPC), advanced assessment skills, non-medical prescribing, and relevant experience in urgent/emergency care.

    Salary: £49,725 per year (FTE), including weekend enhancements.
