Our Complex Lives business unit provides healthcare to those who are particularly vulnerable across Blackpool, Fylde, Wyre, and beyond.
Hours: 16 hours per week
Service Operates: Monday to Friday between
Pay: £27.49 - £30.63 depending on qualifications and experience

Job responsibilities
Duties and Responsibilities
- Practice autonomously as an independent practitioner/prescriber, undertaking assessment, diagnosis, treatment/referral, and discharge of patients presenting with minor illness or injury within agreed parameters.
- Support the Management team to ensure all QUOF Targets are met.
- Provide support for junior staff members.
- Work closely with the management team to develop and support new opportunities.
- Provide advanced clinical skills and advice, demonstrating understanding of issues related to identification, assessment, diagnosis, treatment, and management of a client group while working autonomously.
- Identify the need for and interpret relevant diagnostic investigations and results.
- Safely and competently undertake invasive and non-invasive procedures to establish a diagnosis.
- Work within current nurse prescribing legislation.
- Lead improvements to standards of care through supervision, clinical audits, evidence-based practice, teaching, and professional leadership.
- Act in accordance with the NMC Code of Professional Conduct.
- Provide professional and clinical advice within the multidisciplinary team.
- Maintain accurate, complete records of patient consultations, compliant with legislation, policies, and procedures.
- Prioritize and manage workload to maintain quality.
- Deliver care according to guidelines.
- Obtain and document consent for treatments, ensuring patient participation in decision-making.
- Adhere to policies on infection control, chaperoning, and risk management.
- Fulfil safeguarding responsibilities for adults and vulnerable groups, reporting concerns appropriately.
- Perform other duties as assigned by the Head of Nursing for organizational benefit.
Clinical practice
- Assess, diagnose, plan, implement, and evaluate treatment for patients with undifferentiated and long-term conditions, following policies and guidelines.
- Manage acute and chronic conditions, integrating drug and non-drug treatments.
- Prescribe medications appropriately, adhering to evidence-based practice and protocols.
- Support patient adherence and provide medication information.
- Promote health and well-being through education, screening, and communication.
- Communicate sensitive information effectively to patients, relatives, and carers.
- Lead mental health reviews and assessments, collaborating with pharmacists as needed.
- Refer patients to other healthcare providers when necessary.
- Conduct comprehensive assessments from psychological and physiological perspectives.
- Use clinical templates, read codes, and protocols correctly.
- Report incidents such as falls, aggression, safeguarding concerns.
