Integrated Community Coordination Teams - Whilst working in the ICCT s you will be completing strengths-based
Care Act assessments and person-centred support planning with a wide variety of service user groups aged 18 and
over. The work is diverse, and the aim is to promote the well being of adults with care and support needs to
continuously improve their outcomes, creating lasting connections with their local community.
